In Kirklees we aim to deliver high quality personalised services and are looking for a Community Assessment Support Officer who is passionate about delivering the outcomes of personalisation. Is that you?
You will be required to consider holistic assessments and reviews of a person’s needs, and this will involve seeking and encouraging the contribution of customers, carers and relatives, as well as others involved in their lives and in the provision of their care, including health professionals. A high-quality strength based, and customer focused approach is expected, and is essential in ensuring positive, person centred outcomes are achieved.

Travel is an essential part of this job and therefore it would be extremely difficult for you to do your job effectively if you did not use a car for all or most of the time. It is essential that you hold a full and valid driving licence, and it is expected that you will either use a council vehicle or your own car. Exceptions can be made for disabled applicants.
