Community Care Officer

Community Care Officer (Fixed Term, Part Time, Hybrid)


Salary & Benefits

  • Salary: £30,024 - £33,699 per annum (pro rata)
  • Hours: Part-time (22.5 hours per week) for 12 months
  • Location: Meadow House, Mansfield, NG18 2TB
  • Working Arrangements: Hybrid
  • Competency Framework: Tier 7a

Responsibilities

Your key duties will include:

  • Identifying Goals: Collaborate with clients to understand and realise their aspirations.
  • Coordinating Care: Arrange and manage services to meet individual needs effectively.
  • Promoting Independence: Empower clients to take control of their own care.
  • Collaborating with Professionals: Work seamlessly with multidisciplinary teams to provide holistic support.
  • Managing Caseloads: Balance client needs efficiently to ensure high-quality, timely care.

Requirements

To succeed in this role, you should have:

  • Relevant Qualification: In social care or a related field.
  • Experience: At least 2 years in adult social care (preferably in a community setting).
  • Care Approaches: Knowledge and application of strength-based and person-centred care.
