Key Responsibilities
- Identify Client Goals: Work closely with clients to discover their personal aspirations and outcomes.
- Coordinate Care Plans: Organise a range of support solutions to meet individual needs.
- Promote Independence: Encourage clients to manage their own care and risks proactively.
- Collaborate with Professionals: Partner with other healthcare and social care practitioners to deliver comprehensive support.
- Manage Caseloads: Ensure timely, high-quality support by effectively balancing multiple client cases.

Requirements & Skills
Essential Qualifications & Experience:
- A relevant qualification in social care or a related field.
- 2+ years of experience in adult social care or a comparable role.
- Familiarity with strength-based and person-centred care approaches.
- Knowledge of adult social care policies and processes.

- Empathy and Compassion: A genuine dedication to championing clients’ dignity and needs.
- Strong Communication: Active listening and clear, professional interaction with clients and colleagues.
- Problem-Solving: Proactive approach to addressing challenges and shaping client-centred solutions.
- Teamwork: Ability to collaborate effectively with social care professionals, agencies, and families.
- Organisational Skills: Capacity to manage multiple caseloads efficiently.
