About the Role
As a Community Care Officer (CCO), your responsibilities include:
- Responding to social care needs in alignment with the Care Act 2014 and Mental Capacity Act 2005.
- Supporting individuals by promoting inclusive community care, encouraging a self-directed approach, and maximising universal services.
- Thriving as part of a small, collaborative team, with an emphasis on resilience, imagination, and reflection.
Team-Specific Roles & Responsibilities
1. Integrated Discharge Team – Barnet & Chase Farm Hospital (Permanent)
- Hours: 37h/week (including 1 weekend shift every 8 weeks)
- Core tasks:
- Conduct assessments for safe, timely hospital discharges.
- Collaborate with hospital discharge teams and care providers.
- Implement person-centred care using Hertfordshire’s Connected Lives principles.
- Manage competing priorities, prioritise with sound judgement, and support vulnerable individuals return home independently.

2. Transfer of Care Teams (Fixed term – all in Stevenage)
- Includes the Rehab Team, Assessment & Review Team, and Emergency Discharge Team.
- Hours: 37h/week
- Key responsibilities:
- Perform social care assessments to ensure safe, timely discharges.
- Adhere to Hertfordshire’s Connected Lives Practice Principles for independence-focused support.
- Work closely with hospital staff and care providers for care planning.
3. Older People West – Area Team West (Hemel Hempstead, Fixed term until May 2027)
- Roles under Care Act:
- Assess, safeguard, plan, and review support for adults with long-term health/social care needs.

4. Watford General Hospital Integrated Discharge Team (Permanent)
- Hours: 37h/week (with 1-2 weekend shifts every 8 weeks – typically Saturday/Sunday).
- Role focus:
- 7-day discharge service to enable quick, safe hospital departures.
- Person-centred approach, primarily working via Discharge to Assess.
- No remote/hybrid options – office/hospital-based only (9am–5pm daily).
About You (Essential Criteria)
- Full UK driving licence (regular access to a vehicle required for community duties).
- Experience in social care (preferably in a related field).
- English fluency (ability to converse confidently with the public).
- Right to work in the UK ( evidenced by established documents).
- Temporary isn’t able to sponsor for this role.
