- £30,024 - £33,699 per annum (pro rata)

- Work closely with clients to understand their personal goals and aspirations.
- Use strength-based approaches to promote choice, control, and self-directed support.
- Encourage independence and help clients manage their own care and risks.
You’ll thrive in a fast-paced, supportive team while developing strong organisational and caseload management skills.
Key Responsibilities
- Identify and assist clients to achieve their personal goals.
- Coordinate care plans tailored to individual needs.
- Promote independence by enabling clients to take control of their own support.
- Collaborate with professionals (social workers, healthcare providers, etc.) to deliver holistic care.
- Effectively manage a caseload to ensure high-quality, timely support.

Requirements & Qualifications
- A relevant qualification in social care or a related field.
- 2 years’ experience in adult social care (preferably in a similar role).
- Knowledge of strength-based approaches and person-centred care.
- Experience working in adult social care environments.
Key Skills & Attributes
You’ll need to demonstrate:
- Empathy and compassion for those you support.
- Strong communication skills (listening and clarifying information).
- Problem-solving abilities to address client challenges proactively.
- A collaborative approach when working with multi-disciplinary teams.
