Community Care Officer (Fixed Term, Part Time – 22.5 hrs/week, Hybrid)
Salary Information
£30,024 - £33,699 per annum (pro rata)

As a Community Care Officer, you will help clients define and achieve their personal goals by aligning support around what is meaningful to them. We foster choice, control, and self-directed support, enabling individuals to assess and manage their own needs and risks effectively.

Key Responsibilities
- Identify clients' personal goals and aspirations and work collaboratively to achieve them
- Coordinate care by organising outcomes tailored to individual needs
- Promote independence, helping clients take control of their own care
- Collaborate with professionals (e.g., health workers, specialists) to provide comprehensive support
- Manage caseloads effectively, ensuring timely and high-quality assistance

Requirements & Experience
Essential Qualifications & Background
- Relevant qualification in social care or a related field
- Experience in adult social care or a comparable environment
- Strong knowledge of strength-based approaches and person-centred care
- 2 years of adult care experience (minimum)
