What is the job role?

We have exciting Community Case Worker posts within our One Coventry Integrated Team. You will be supporting people who have a wide range of social care issues. Reducing hospital admissions and improving the flow out of the hospital. There is a real emphasis on promoting independence. This is a great opportunity for motivated individuals who enjoy delivering person-centred support.

This post is exempted under the Rehabilitation of Offenders Act 1974 and as such appointment to this post will be conditional upon the receipt of a satisfactory response to a check of police records via Disclosure and Barring Service (DBS).

Who are we looking for?

We are looking for a person with a passion for promoting independence and making the principles of the Care Act 2014, a reality for the people of Coventry.

If you like to work as part of a team but can also use initiative and work autonomously, we would really like to hear from you.

As the successful candidate you will have:

Experience of assessment and case management work with older people, people with physical disability, people with learning disability or other adult service users. Have excellent communication skills. Ideally have experience of recording and record keeping procedures. Experience of team membership and participation. Experience of working with a range of service user groups either in a personal or professional capacity. Have very good organisational skills.
