Community Children's Nurse
Permanent Band 5 Staff Nurse – Children’s Community Nursing (CCN)
About the Role
The Children’s Community Nursing (CCN) service is seeking an enthusiastic and highly motivated Band 5 Staff Nurse to join the Integrated Children’s Nursing and Therapies (ICNT) team. This role focuses on delivering holistic nursing care to children and young people in their homes or community settings, with the primary aim of:
- Delivering clinical interventions
- Preventing hospital admissions
- Supporting early discharges
- Teaching families to manage their child’s healthcare needs independently
You’ll assess, plan, implement, and evaluate individualised nursing care plans for children with acute, chronic, or life-limiting conditions. Working both autonomously and collaboratively, you’ll engage with multidisciplinary team (MDT) meetings—including discharge planning, transition, and safeguarding.
You’ll be joining an approachable, motivated, and supportive ICNT team, providing high-quality care in primary and community settings across the Lancaster, Morecambe, and Carnforth areas.
Responsibilities
Core Duties Include:
- Assess, plan, implement, and evaluate evidence-based individualised nursing care plans for children with acute, chronic, or life-limiting conditions in home or community settings.
- Develop and maintain competencies in clinical skills relevant to the designated area of practice.
- Provide family support and education, including:
- Training and guidance for parents, carers, and family members
- Feedback and advice for multidisciplinary teams and external stakeholders (e.g., education staff)
- Actively contribute to:
- Transition and discharge planning
- Safeguarding assessment, documentation, and reporting
- Referrals to other professionals or agencies as needed
- The statutory Education, Health and Care Plan (EHCP) processes
- Ensure privacy, dignity, and individualised care, making reasonable adjustments where necessary.
- Participate in reflective learning, continuing professional development (CPD), and revalidation in line with professional requirements.

The Environment
Our nursing and therapy services are delivered across:

Three main hospitals:
- Furness General Hospital (FGH), Barrow-in-Furness
- Royal Lancaster Infirmary (RLI), Lancaster
- Westmorland General Hospital (WGH), Kendal
Each provides a range of inpatient, outpatient, diagnostics, therapies, day case, and inpatient surgery services.

- Ulverston Community Health Centre

Who We Are
Our Integrated Children’s Nursing and Therapies (ICNT) team serves Lancaster, Morecambe, and Carnforth. The CCN role is currently based at the Royal Lancaster Infirmary.
Requirements
- Band 5 registered nurse qualification
- Enthusiastic, empathetic, and highly motivated to provide individualised paediatric care
- Exceptional communication skills, with the ability to:
- Build trust with children
- Reassure and support anxious families
- Ability to work autonomously and collaboratively in an MDT environment
- Commitment to safe-guarding, reflective practice, and professional development
