Advanced Clinical Practitioner (Community Frailty Service) – Full-Time
Band 8a, Fylde Coast Community Frailty Team
About the Role
This is a full-time (B8a) position within a multidisciplinary team led by a Consultant Geriatrician, supporting patients across the Fylde Coast to live well alongside long-term conditions.
The Community Frailty Service offers:
- Comprehensive Geriatric Assessments followed by up to 12 weeks of individualised care planning
- The Frailty Virtual Ward, providing remote monitoring and support
- Advice & Guidance to both Primary and Secondary care providers
The team comprises:
- Frailty GPs
- Nurse Consultants
- Advanced Clinical Practitioners
- Frailty Nurses
- Pharmacy team
- Support staff
The role is based at Moor Park Health & Leisure Centre and South Shore Primary Care Centre, operating on a rotational basis across the wider area.
Key Responsibilities
Your duties will include:
Clinical Care & Assessment
- Receive and assess patients with undiagnosed or undifferentiated health concerns.
- Conduct highly specialised clinical assessments aligned with frailty aspects and long-term condition management.
- Screen and evaluate early disease indicators, identifying potential health risks.
- Develop differential diagnoses, showcasing strong decision-making and problem-solving skills.
- Lead ongoing care plans that focus on education, prevention, and promoting health and well-being.

Care Management & Referrals
- Order investigations and implement treatments, working both individually, collaboratively, and via inter-agency referrals.
- Support patients to remain safely at home through proactive frailty management and virtual ward care.
- Provide suppport in managing long-term illness as part of a holistic approach.
- Hold authority over admission, discharge decisions, and CAD referrals within your caseload.
Service Delivery & Collaboration
- Deliver a daily telephone triage service, home acute visits, and comprehensive geriatric assessment clinics.
- Oversee all caseload tasks, including requesting and managing investigations.
- Demonstrate leadership by providing consultancy and expertise in geriatric care as required.

Prescribing & Administration
- Utilise the GP EMIS electronic record for prescribing and care documentation.
Requirements
- Registered with the NMC (Nurses or allied health professionals).
- Highly developed clinical skills and frailty knowledge.
- Strong decision-making and clinical leadership abilities.
- Experience in gestaltmedicine and holistic care planning.
- Ability to work autonomously while collaborating in multidisciplinary settings.
