Within the Community Frailty Service (Fylde Coast, UK)
About the Role
- Location: Moor Park Health & Leisure Centre and South Shore Primary Care Centre (rotational basis)
- Employment Type: Full-Time, Band 8a
- Team Structure: Multidisciplinary team led by a Consultant Geriatrician, including:
- Frailty GPs
- Nurse Consultants
- Advanced Clinical Practitioners (ACPs)
- Frailty Nurses
- Pharmacy team
- Support staff
Service Overview
Our Community Frailty Service supports patients with long-term conditions to live well with frailty. Following a Comprehensive Geriatric Assessment (CGA), patients receive up to 12 weekly sessions with an individualised care plan.
We also deliver:
- The Frailty Virtual Ward
- Advice & Guidance to Primary and Secondary Care teams
Key Responsibilities
Your role will include:
- Patient Assessment & Care Planning
- Work with patients presenting with undifferentiated or undiagnosed problems
- Conduct advanced clinical assessments to determine health care needs
- Screen for disease factors and early illness signs
- Develop and implement ongoing care plans, emphasising health education and preventative measures

Investigation, Treatment & Coordination
- Order necessary investigations and deliver tailored treatment
- Manage admissions/discharges for patients on your caseload
- Refer to other healthcare and social care providers as needed

Proactive Frailty Management
- Support patients to remain safe at home through virtual ward care
- Lead patients in self-management of chronic illness and frailty

Collaboration & Leadership
- Work collaboratively with GPs, nurses, pharmacists and social care professionals
- Provide consultancy and mentorship as required

Daily Duties
- Conduct daily telephone triage
- Carry out home acute visits
- Lead and attend comprehensive geriatric assessment (CGA) clinics
- Oversee and manage caseload investigations
Prescribing Requirements
- Use EMIS electronic prescribing system (Standard GP functionality)
Referrals
- The service accepts referrals from all health and social care professionals across the Fylde Coast geographies.
About the Community Frailty Service
Our team operates across the entire Fylde Coast area, dedicated to improving frailty care for older adults. We work within a GP-led model, complementing existing primary and secondary care services.
