Thame Adult Community Health Care Team (ACHT) Job Opportunity
Health Care Assistant Post
As a member of the Adult Community Healthcare Team, the post holder will be providing care alongside nurses and nursing associates within the district nursing team.
Overall Aims
The overall aims of the Adult Community Healthcare Team are to:
- Avoid inappropriate admission into the acute service
- Facilitate timely discharge into the community
- Enable individuals to be cared for in their home environment
Responsibilities
The post holder will be appropriately trained and supervised in various skills to work independently supporting the District nursing service.
- Work in the community, supporting community nurses in providing nursing care interventions
- Promote independence in the patient’s home environment
Requirements
This is an exciting and innovative service. Candidates must be experienced carers and be able to demonstrate having experience or knowledge of supporting patients in their own homes. You will have excellent communication skills and the ability to work with a range of health professionals and informal carers. The ideal candidate will be flexible and adaptable, being able to respond to need at short notice.

- Experience and NVQ level 3 or equivalent in Health and Social Care
Working Conditions
In order to respond to the healthcare needs of the local population, the service covers a 7 days a week rota which therefore includes weekends and bank holidays.

The ACHT reflects an integrated approach that has been designed in response to the changing needs from the local population, with a special emphasis being placed on the proactive vision of care for patients in the community setting.
