Thame Adult Community Health Care Team (ACHT) Job Opportunity

About the Role

Thame Adult Community Health Care Team (ACHT) has an exciting job opportunity for a Health Care Assistant post.

As a member of the Adult Community Healthcare Team, the post holder will be providing care alongside nurses and nursing associates within the district nursing team.

Aims of the Adult Community Healthcare Team

The overall aims of the Adult Community Healthcare Team are to:

  • Avoid inappropriate admission into the acute service
  • Facilitate timely discharge into the community
  • Enable individuals to be cared for in their home environment

Responsibilities

The post holder will be appropriately trained and supervised in various skills to work independently supporting the District nursing service. The post will involve working in the community, supporting the community nurses in providing nursing care interventions, and promoting independence in the patient’s home environment.

Requirements

This is an exciting and innovative service. Candidates should be experienced carers and be able to demonstrate having experience or knowledge of supporting patients in their own homes. You will have excellent communication skills and the ability to work with a range of health professionals and informal carers. The ideal candidate will be flexible and adaptable, being able to respond to need at short notice.

The services cover a 7 days a week rota which therefore includes weekends and bank holidays. Ideally, the candidate will have experience and must hold a NVQ level 3 or equivalent in Health and Social Care.
