At Thrive Tribe we deliver award-winning healthy lifestyle and wellbeing programmes that make people happier and healthier! It’s a super exciting time to join us as we begin to deliver our Integrated Health and Wellbeing Service (IHWS) throughout the City of Nottingham, and we’re looking for a team of Community Health Coaches to join us!
Our IHWS programme is designed to support individuals and families to improve their health behaviours and make sustainable changes to their lifestyle. The programme consists of five key services:
- Stop smoking support
- Tailored physical activity programmes
- Adult weight management
- Child/family weight management
- Health Coaching (this is where you come in!)
The purpose of a health coach is to improve lives, through empowering people to make effective long-term changes. This is achieved through successfully delivering our quality programmes and providing evidence-based information, supporting individuals to improve their overall health and wellbeing (including moving more, eating better, stopping smoking and providing onwards referrals where appropriate, such as for mental wellbeing support!).

A typical day as a Community Health Coach might include:
- Motivating, inspiring and supporting individuals to adopt a healthier lifestyle, through a professional, knowledgeable and empathetic approach
- Delivering evidence-based interventions that focus on diet, nutrition, physical activity, smoking cessation and behaviour change
- Co-producing personalised plans with individuals, empowering them to take control of their own healthy lifestyle choices, and providing information and resources that support them to achieve their desired outcomes
- Ensuring an excellent and safe experience for clients, aligning with our internal policies and procedures
- Referring into our other pathways and where appropriate signposting to external support services
- Engaging in ongoing training relevant to your role, including submitting your annual CPD portfolio and evidencing professional development in line with our CPD policy
- Working with colleagues across all of our Thrive Tribe services to contribute to new projects and service developments – we’re always looking to share knowledge and resources!
- Ensuring due consideration is given to Information Governance and Clinical Governance legislation, requirements and guidance
- Helping us maintain the ethos of Thrive Tribe by driving our organisational culture forward across our services and using every opportunity to embed our values!

We’re looking for someone who can combine health improvement knowledge with first-class communication and motivational skills. If you have experience of delivering health interventions, and a real understanding of the impact personal characteristics and circumstances can have on health behaviours then we want to hear from you. Bonus points if you have a qualification in a health promotion related subject, such as nutrition, exercise, or health coaching!
Although not essential, so that we can best support the needs of our clients in Nottingham we are particularly keen to hear from applicants who are fluent in Polish, Urdu, Arabic and/or Romanian.

Contract:
Part Time (22.5 hours), permanent position available. We are keen to achieve a spread across the City of Nottingham to enable us to target communities in our place-based health coaching approach. We are looking for individuals who can cover the following areas;
- Bulwell
- Top Valley and Bestwood
- Bilborough and Broxtowe
- Aspley and Whitemoor
- New Basford, Hyson Green and Radford
- St. Ann’s
- The Meadows and Sneinton
- Clifton
Salary:
£14,871 for this part time, 0.6 contract

Location:
This role is based in the City of Nottingham, with regular travel across the patch (car owner preferred, but not mandatory). The job will involve some home/office working when not delivering face-to-face sessions.
