Are you someone who thrives on helping others and making a real impact every day? As a Community Health Navigator, you’ll be at the heart of our service—the friendly, knowledgeable voice that connects people to the right care at the right time.
Based in Chippenham or Swindon, you’ll join our BSW (Bath & North East Somerset, Swindon and Wiltshire) Community Health Services Single Point of Access (SPA)—a fast-paced, supportive hub where no two days are the same. You’ll be the first point of contact for patients, the public, healthcare professionals, and colleagues, providing reassurance, guidance, and solutions when they matter most.
Through phone, email, and written communication, you’ll deliver a service that is efficient, professional, compassionate, and person-centred. You’ll help people navigate services, ensure referrals are handled smoothly, and play a vital role in keeping care flowing where it’s needed most.
This is a dynamic frontline role where your communication skills, attention to detail, and ability to stay calm under pressure will truly shine. If you enjoy working in a busy environment, solving problems, and being part of a team that genuinely supports one another—this is the role for you.
Remote working is available following successful completion of the probationary period.
What You’ll Be Doing
- Be the welcoming first point of contact for all SPA enquiries
- Support callers with empathy and professionalism, identifying needs and directing them to the right services
- Handle sensitive and sometimes complex information with confidentiality and care
- Keep accurate, up-to-date patient records across clinical systems
- Process referrals and correspondence efficiently and accurately
- Book, amend, and manage clinical appointments in line with service targets
- Escalate urgent or incomplete referrals appropriately to clinical teams
- Provide essential admin support to keep services running smoothly
- Work collaboratively with a friendly, supportive team, contributing to shared goals

Working Patterns
- The SPA operates 8:00 AM to 8:00 PM, 7 days a week, 365 days a year
- Flexible shift patterns available to accommodate service needs while supporting a healthy work–life balance
Essential Criteria
- GCSEs (or equivalent) in Maths and English at Grade A–C
- Previous administrative experience
- Confident IT system usage, strong keyboard skills, and attention to accuracy
- Good working knowledge of Microsoft Office (Outlook, Word, Excel)
- Strong communication and interpersonal skills, able to convey messages clearly, both verbally and in writing
- Ability to work with sensitivity, discretion, and confidentiality
- Well-organised, able to prioritise tasks, meet deadlines, and work effectively in a busy environment
- Ability to work independently and as part of a team

Desirable Criteria
- NVQ Level 2 in Administration and/or Customer Services (or equivalent)
- Previous experience within health or social care
- Knowledge of medical terminology
- Experience using clinical systems or databases
