Community Services – Birmingham Hospice

- Hours: Full Time (37.5 hours per week)
- Location: Community Services (Cross-site working across Birmingham Hospice locations)
- Salary: Hospice Clinical Band 8a £53,270 - £59,958 (dependent on experience)
- Contract: Permanent
- DBS: Enhanced with Adult Barred List
Lead Compassionate Community Care

We are looking for an experienced clinical leader to join our senior leadership team as Community Services Lead, shaping and delivering high-quality specialist palliative care across our integrated community services.
This is an exciting opportunity for an experienced senior nurse who is passionate about leading people, developing services, and improving the experience of patients and families living with life-limiting illness.
About the Role
Reporting to the Head of Community Services, you will provide operational and clinical leadership for our Integrated Community Team, including:
- Clinical Nurse Specialists
- Hospice at Home
- Specialist Palliative Urgent Response SPUR
- CNS Triage
You will ensure the safe, effective, and responsive delivery of specialist palliative care services while supporting service transformation, workforce development, and quality improvement. As a visible clinical leader, you will play a key role in delivering Birmingham Hospice's strategic ambitions for community-based palliative care. Working collaboratively with multidisciplinary colleagues, Integrated Neighbourhood Teams (INTs), NHS partners, primary care, social care, and the voluntary sector, you will lead the co-design and development of innovative, person-centred models of care that improve access, reduce inequalities, and respond to the changing needs of our communities. You will champion partnership working across the wider health and care system, ensuring hospice expertise is embedded within integrated pathways and neighbourhood-based care.

What You’ll Do
- Lead and inspire multidisciplinary community teams to deliver outstanding specialist palliative and end-of-life care.
- Provide expert clinical leadership for patients with complex palliative care needs.
- Manage service capacity, workforce planning, and operational performance.
- Support, coach, and develop clinical teams through supervision, appraisal, and professional development.
- Lead quality improvement, clinical governance, audit, and service evaluation.
- Drive the delivery of the Hospice's community services strategy through innovation, service transformation, and continuous improvement.
- Co-design and implement new models of care with patients, families, Integrated Neighbourhood Teams (INTs), and system partners to improve patient experience, equity of access, and outcomes.
- Build and sustain strong relationships across the wider health and care system, including NHS, primary care, community, social care, and voluntary sector organisations, representing Birmingham Hospice as a trusted strategic partner.
- Promote a culture of compassion, innovation, learning, and continuous improvement.
About You
We are looking for an experienced senior nurse who can demonstrate:

- Registered Adult Nurse with current NMC registration.
- Significant experience in specialist palliative or end-of-life care.
- Proven leadership and management experience within community, hospice, or acute services.
- Experience of leading strategic service development, innovation, and transformational change.
- A strong understanding of integrated working, including collaboration with Integrated Neighbourhood Teams (INTs) and wider health and care system partners.
- Excellent communication, coaching, and relationship-building skills, with the ability to influence across organisational boundaries.
- Experience of leading service improvement, quality initiatives, and co-design with patients, carers, and partners.
- Sound understanding of clinical governance, safeguarding, and risk management.
- Non-Medical Prescribing qualification (or willingness to develop if appropriate).
- Ability to work collaboratively across organisational and professional boundaries.
