North Doncaster Community Learning Disability Team
The North Doncaster Community Learning Disability Team is a specialist health care team working to meet the health care needs of adults with learning disabilities.
We are looking to recruit a motivated and enthusiastic practitioner into this challenging and rewarding post.
About the Role
Job Title: Learning Disability Practitioner
Reports to: Senior Community Nurse
You will be responsible for:
- Assessing and supporting the health needs of adults with a learning disability
- Managing a defined caseload requiring tailored support from the service
- Conducting assessments to determine the appropriate level of support and intervention
- Implementing evidence-based interventions following assessments
- Monitoring and reviewing the effectiveness of interventions
- Collaborating with the multi-disciplinary team, including individuals, families, professionals, and carers, to ensure a holistic approach
- Working effectively as part of an established team, while also demonstrating autonomy when appropriate
Responsibilities & Key Activities
- Complete assessments to evaluate health needs and intervention requirements
- Develop individualised care plans based on assessment outcomes
- Ensure interventions are evidence-based and aligned with health best practices
- Engage with service users, carers, and professionals to deliver comprehensive support
- Participate in team discussions, reviews, and service planning
- Work flexibly, adapting to the needs of diverse service users
- Maintain accurate, up-to-date records to comply with service and regulatory standards

Requirements & Skills
Required qualifications and attributes include:
- Marinating occupied caseload and independently supporting service users directly in their own homes and community settings
- Must have current registration with the NMC (professional register), which is valid and maintained
- Successful completion of a learning disability nursing program relevant to Nursing and Midwifery Council registration
- Independent self-management and leadership skills
- Valid and regularly updated CPR, DCV, EPR competency by the date of appointment, available upon request
- Ample experience in adults with learning disabilities to encompass the in-depth learning and development.
