Community Learning Disability Nurse

Registered Health Professional – Community Nurse (Learning Disability)

Essex Learning Disability Partnership are seeking a Registered Health Professional with a passion for delivering the highest standard of care to individuals with a learning disability, working within the North East Essex Community Learning Disability Team based at Lexden Site, Colchester.


About the Role

The Northeast Essex Community Learning Disability Team provides person-centred and holistic assessment and care for adults with a learning disability. This includes:

  • Supporting individuals living independently in the community
  • Working with families/carers
  • Providing care for those in supported living schemes or 24-hour residential placements

The team is committed to improving the health and well-being of people with a learning disability by empowering individuals to maximise their potential through a simple, person-centred approach.


Key Responsibilities

  • Assess the healthcare needs of individuals with learning disabilities across various settings and implement evidence-based interventions.
  • Work interdisciplinarily to evaluate service user needs, develop care plans, and measure outcomes.
  • Engage directly with service users, carers, colleagues, and external service providers.
  • Promote health inclusion and ensure equal access to mainstream health services, adhering to national and local healthcare guidelines.
  • Deliver training and education to professionals, carers, and families on learning disability-related health needs.

Professional Requirements

  • Registered with the Nursing and Midwifery Council (NMC)
  • Compassionate, resilient, and dedicated to empowering service users through independence support

Key Competencies

  • Effective communication skills to engage with:
    • Service users
    • Multidisciplinary team (MDT) colleagues
    • Carers/family members
    • External partners involved in care
  • Ability to maintain high standards of record keeping
  • Strong understanding of legislation and national guidance in learning disability care and advocacy
