Community Learning Disability Nurse

Band 6 Community Learning Disability Nurse – Gwynedd Learning Disability Team

Job Overview

This is an exciting opportunity to join the Gwynedd Learning Disability Community Health Team at Betsi Cadwaladr University Health Board.

As a Band 6 Community Learning Disability Nurse, you will work within the Learning Disability team based in Gwynedd, ensuring collaboration between the Local Authority, neighbouring services (including community mental health teams, inpatient units), and statutory/voluntary sector agencies.

Your role will involve delivering evidence-based, safe, and effective clinical interventions, following best practice in nursing care.


Main Responsibilities

  • Ensure delivery of specialist learning disability services aligned with the service strategy and agreed national standards, maintaining safe, high-quality care.

  • Provide excellent communication skills, acting as a visible, accessible member of the team for patients, families, and colleagues. Offer support, advice, and assistance to those in need.

  • Handle highly sensitive and complex information, analysing, interpreting, and responding appropriately—including navigating potential barriers to acceptance.

  • Demonstrate a strong understanding of the Community Health Cohort (CHC) process, with active involvement in the development of pathways and Maturity Matrix models.

Qualifications & Knowledge

Essential Criteria

✓ RNLD Registration (registered as a Learning Disability Nurse with the NLM/V) or equivalent qualification. ✓ Degree-level qualification (or equivalent) in line with NMC registration. ✓ Evidence of postgraduate education and ongoing Continuous Professional Development (CPD).

Experience

Essential Criteria

🔸 Significant clinical experience within learning disability nursing (preferably in community settings) or a relevant nursing specialization.

Desirable Criteria

🔸 Experience working in multidisciplinary teams 🔸 Prior knowledge or experience in a senior nurse role (e.g., lead clinical position)

Essential Criteria

🎯 Ability to:

  • Communicate complex information effectively.
