Community Learning Disability Nurse (Band 6) – Gwynedd Learning Disability Team


Job Overview

This is an exciting opportunity to join the Gwynedd Learning Disability Community Health team at Betsi Cadwaladr University Health Board.

This is a Band 6 Community Learning Disability Nurse position based in Gwynedd. The postholder will work within the Learning Disability team, ensuring collaboration between the Local Authority, neighbouring services, and key statutory and voluntary sector partners. This includes supporting community mental health teams, in-patient services, and more.

The role involves delivering evidence-based, safe, and effective care while adhering to nursing frameworks. Would you like to help shape care pathways and models of service delivery in the region?

Key Responsibilities

  • Work within the service principles and strategy for people with a learning disability, delivering high-quality, specialist care.
  • Demonstrate excellent interpersonal skills, acting as an accessible, empathetic team member.
  • Handle and interpret complex, confidential, and sensitive information, aligning support with identified barriers.
  • Develop and review CHC processes, pathways, and Maturity Matrices.
  • Foster strong relationships with patients, families, and multidisciplinary teams.

About the Role

Main Duties & Responsibilities

✔ Deliver evidenced-based care in line with HCPC, NMC, and national standards. ✔ Coordinate effectively across community and statutory services. ✔ Promote person-centred planning, ensuring holistic support. ✔ Collaborate with multi-disciplinary teams to enhance care pathways. ✔ Support safeguarding protocols and advocate for patients’ rights.


Person Specification & Requirements

Qualifications & Knowledge

Essential Criteria

  • Registered Nurse (Adult, Child, or Mental Health LD) with the NMC and authorized to practice.
  • Must hold a degree-level Qualification (or equivalent) relevant to nursing, including:
    • Postgraduate education (e.g., Masters or further specialist training).
    • **Evidence of Continuing Professional Development (CPD) relevant to learning disability nursing.

Desirable Criteria

  • Management certification (e.g., motivational leadership, supervision, or mentoring).
  • Advanced mentorship or training (for aspiring supervisors/assessors).

Experience

Essential Criteria

  • Extensive clinical experience in Learning Disability nursing (or equivalent specialist area).
  • Demonstrated ability to work autonomously and within collaborative multispeciality settings.

Desirable Criteria

  • Measurable experience supporting senior roles (e.g. overseeing pathways or advocating for policy change).
  • Experience working in MDT environments (e.g. mental health, social care).

  • Specialist diagnostic skills to identify gaps in care and implement interventions.
