Are you a newly qualified Learning Disabilities Nurse? Do you want to work with a diverse set of clients?
We are seeking a compassionate and motivated Band 5 Community Learning Disability Nurse to join our multidisciplinary team, delivering high-quality, person-centred care to adults with learning disabilities in the community. This is an exciting opportunity to develop specialist skills while making a meaningful difference to individuals’ health, independence, and quality of life.
We have 2 roles available a full-time role with our Abingdon based team and a part-time (22.5 hours per week) role with our Banbury based team. Please indicate your preference in your supporting statement.
In this role, you will work collaboratively with individuals, families, carers, and partner agencies to assess health needs, plan and deliver care, and promote health and wellbeing. You will support service users to access mainstream healthcare services, reduce health inequalities, and maintain independence within their own homes and local communities.

- Deliver safe, effective, and person-centred nursing care within the community setting
- Undertake holistic assessments and develop, implement, and evaluate care plans
- Promote health education and support individuals to make informed choices
- Work in partnership with families, carers, and other professionals
- Support access to primary and secondary healthcare services
- Participate in safeguarding processes and risk management
- Maintain accurate clinical records and contribute to service improvement
