Job summary

Are you a newly qualified Learning Disabilities Nurse? Do you want to work with a diverse set of clients?

We are seeking a compassionate and motivated Band 5 Community Learning Disability Nurse to join our multidisciplinary team, delivering high-quality, person-centred care to adults with learning disabilities in the community. This is an exciting opportunity to develop specialist skills while making a meaningful difference to individuals' health, independence, and quality of life.

We have 2 roles available: a full-time role with our Abingdon based team and a part-time (22.5 hours per week) role with our Banbury based team. Please indicate your preference in your supporting statement.

In this role, you will work collaboratively with individuals, families, carers, and partner agencies to assess health needs, plan and deliver care, and promote health and wellbeing. You will support service users to access mainstream healthcare services, reduce health inequalities, and maintain independence within their own homes and local communities.

Main duties of the job

  • Deliver safe, effective, and person-centred nursing care within the community setting
  • Undertake holistic assessments and develop, implement, and evaluate care plans
  • Promote health education and support individuals to make informed choices
  • Work in partnership with families, carers, and other professionals
  • Support access to primary and secondary healthcare services
  • Participate in safeguarding processes and risk management
  • Maintain accurate clinical records and contribute to service improvement

Job Description

Job responsibilities

As a key member of the community team, you will contribute to the delivery of evidence-based interventions, including health assessments, positive behaviour support, and physical health monitoring. You will also play an important role in safeguarding, risk assessment, and supporting individuals during periods of change or crisis.

You will receive structured supervision, preceptorship (if newly qualified), and ongoing professional development opportunities, enabling you to build confidence and progress within community learning disability nursing.

Applicants must have a full valid driving licence and have access to a vehicle for work. Adjustments can be considered in line with the Equality Act 2010 where required. Please note if you are based outside of the UK, you will need to be fully qualified, have already passed your OSCE, and have an NMC pin number in order to be considered for this role. Person Specification

Qualifications

Essential

  • Registered Learning Disabilities Nurse with evidence of knowledge and experience of supporting people with learning disabilities with their health needs.

Knowledge

Essential

  • Computer skills including Microsoft systems, electronic patient record systems, virtual consultations.
  • Knowledge and experience of a variety of assessments and interventions.
  • Knowledge of evidence-based practice in clinical areas of practice including Epilepsy; Dementia, physical and mental health; autism and behaviours of concern.
  • Working knowledge of the application of the NMC code of conduct.

Experience

Essential

  • Experience working with people with a learning disability and autistic people.

Skills

Essential

  • Ability to work autonomously.
  • Evidence of effective interpersonal skills (active listening, building empathy).
