Community Learning Disability Nurse

Community Learning Disability Nurse – Band 6

Gwynedd Learning Disability Community Health Team Betsi Cadwaladr University Health Board

About the Role

This is an exciting opportunity to join the Gwynedd Learning Disability Community Health team at Betsi Cadwaladr University Health Board (BCUHB) in Band 6.

The successful candidate will work as part of the Learning Disability Nursing team, ensuring seamless interface with the Local Authority, community mental health teams, inpatient services, and other statutory agencies and voluntary sector organisations within Gwynedd.

The role focuses on delivering evidence-based, safe, and clinically effective interventions through rigorous adherence to the nursing process.


Key Responsibilities

  • Deliver specialist care: Provide high-quality, safe learning disability nursing services in alignment with agreed standards and service strategy.
  • Enhance accessibility: Be a trusted, approachable team member for patients, their families, and carers, offering support, advice, and assistance.
  • Handle sensitive information: Communicate, interpret, and act on complex health and welfare matters, including where clients may encounter barriers to treatment acceptance.
  • Support service development: Contribute to clinical pathways and Maturity Matrix improvements, ensuring best practice in care processes.
  • Multidisciplinary collaboration: Engage effectively with healthcare professionals, social workers, statutory agencies, and voluntary sector partners.

Detailed Job Requirements

Qualifications & Criteria

Essential:

  • Registered Nurse - Learning Disability (RNLD).
  • Degree level qualification (or equivalent) for NMC registration.
  • Evidence of postgraduate education and ongoing Continuous Professional Development (CPD).

Desirable:

  • Management qualifications/experience.
  • Experience as a mentor, supervisor, or assessor.

Experience

Essential:

  • Substantial clinical experience in learning disability nursing or a relevant nursing specialty.

Desirable:

  • Experience working in a multidisciplinary healthcare setting.
  • Understanding or experience of senior nurse roles.

Core Skills & Abilities

Essential:

  • Ability to interpret and act on highly complex or sensitive information.
  • Strong problem-solving and diagnostic skills in patient care planning.
  • Experience working both autonomously and collaboratively in a team.

Desirable:

  • Fluency in spoken Welsh or ability to work effectively in a multilingual environment.
  • IT literacy for managing patient records and service development tasks.
