Community Mental Health Care Coordinator
Community Mental Health Nurse / Occupational Therapist / Social Worker (Reference: [Internal Code]
About the Role
To ensure that adults with mental illness referred to Hillingdon Community Mental Health Hubs (CMHH) receive appropriate assessments, support, and high-quality care within available resources.
- Provide clinical advice, guidance, and support to service users, carers, and other professionals on mental health-related issues.
- Lead holistic assessments, including risk evaluations, mental state examinations, and full needs assessments.
Key Responsibilities

Management & Clinical Work:
- Oversee a caseload, administer medication, and collaborate in MDT meetings.
- Respond daily to duty referrals, prioritising urgent cases.
- Conduct holistic assessments using SystmOne and follow the Single Assessment Process.

Care Coordination & Safeguarding:
- Act as a duty clinician when Care Coordinators/Lead Professionals are unavailable.
- Implement crisis intervention decisions and coordinate emergency support.
- Protect vulnerable adults—recognising and addressing hazard, abuse, or neglect.
- Apply Mental Capacity Act 2005, Deprivation of Liberty Safeguards, and Safeguarding Adults policy.

Documentation & Standards:
- Ensure electronically updated records comply with local policies, NMC, and CNWL guidelines.
- Provide clinical advice to multidisciplinary team members.
- Represent the hub in service quality discussions and audit compliance.

Person Specification
Education & Qualifications
- Mandatory registration as a Registered Mental Health Nurse, Occupational Therapist, or Social Worker.
Experience

Essential:
- Post-registration clinical experience.
- Demonstrated success in a community mental health setting.

Desirable:
- Proven ability to supervise colleagues, staff, and student placements.
Skills & Attributes

Critical Skills:
- Crisis risk management & team contributions: Knowledge of assessments (e.g., MSE, MEE, safeguarding) and evidence-based interventions.
- Assertive Communication: Phone skills, nonverbal cues, and open feedback loops with teams.

Desirable Traits:
- Proactive in audit, service development, and implementation of innovations.
- Strong ability to lead’, mentor, and advocate for professional practice.
