Community Mental Health Nurse

Band 6 | Full Time – 37.5 hours per week
Dementia & Older People's Mental Health Team | Worthing
£39,959 – £48,117 per annum

We're looking for:

  • Registered Mental Health, Adult or Learning Disability Nurses with:
  • Community mental health experience
  • Strong assessment and risk management skills
  • Experience working within multidisciplinary teams and the CPA process
  • Excellent communication and partnership working skills
  • A compassionate, person-centred approach to supporting older adults

The role

As a Community Mental Health Nurse, you'll support older people experiencing dementia and complex mental health needs, delivering assessment, intervention and ongoing care to help individuals remain independent within their communities.

You'll:

  • Manage a caseload of older adults with complex mental health needs
  • Complete specialist mental health and risk assessments
  • Deliver evidence-based, person-centred interventions
  • Support individuals experiencing mental health crises to remain safely at home where possible
  • Work closely with families, carers and partner agencies
  • Participate in the team duty system and multidisciplinary meetings
  • Supervise students and support junior colleagues
  • Contribute to service development and continuous improvement

About the service

The Dementia & Older People's Mental Health Team provides specialist community support across Adur, Arun and Worthing.

The service combines intensive crisis support with ongoing care coordination, helping older adults maintain their wellbeing, independence and quality of life while avoiding unnecessary hospital admission wherever possible.

Location

Based in Worthing, West Sussex, covering the Adur, Arun and Worthing area.

  • Community-based role with travel across the locality
  • Easy access to Brighton, the South Downs and the Sussex coast
