Community Mental Health Nurse

Job Overview

We are seeking a skilled and compassionate Band 6 Registered Mental Health Nurse to join the Queens Park and Paddington Community Mental Health Hub in Westminster.

You will be an integral member of a multidisciplinary community mental health team, providing assessment, treatment and psychosocial interventions to people with a range of mental health needs, supported through recovery focused community care. The role involves holding a defined caseload and working collaboratively with service users, carers and partner agencies to promote recovery, reduce risk and support people to live well in the community.

Main duties of the job

The post holder will take responsibility for the assessment, collaborative formulation, treatment and ongoing review of a defined caseload of people with mental health needs, working in a person centred and recovery focused way.

  • You will deliver psychosocial interventions, or be supported and trained to develop these skills, and contribute to the wider clinical aspects of care including assessment, physical health monitoring, health promotion and healthy lifestyle advice.
  • The role may also include facilitating or co facilitating therapeutic and wellbeing focused groups as part of the hub’s offer.
  • You will undertake comprehensive psychosocial and risk assessments, develop and implement risk management and crisis plans, and work proactively with service users and carers to manage distress, reduce relapse and support people to remain well in the community.
  • You will support medication management, including depot clinics where required, and promote holistic care that recognises the importance of both mental and physical health.
  • You will work closely with multidisciplinary colleagues, inpatient services, Home Treatment Teams, GPs, social care and voluntary sector partners to ensure coordinated and compassionate care.

Detailed Job Description And Main Responsibilities

The post holder will promote fair and equitable access to mental health services, working in a way that recognises and respects diversity, individual identity and lived experience, and supports choice, inclusion and recovery.

  • You will work collaboratively with people, families and carers to understand needs, strengths and goals, and to develop and review personalised, recovery focused plans that support wellbeing, stability and independence.
  • You will deliver evidence based psychosocial interventions and proactive risk management approaches, supporting people to manage distress, reduce relapse and minimise the impact of mental ill health on daily life.
  • You will demonstrate strong safeguarding practice, ensuring safeguarding children and adults procedures are understood and followed, and taking timely and appropriate action when risk or vulnerability is identified.
  • You will provide psychoeducation and health promotion, supporting understanding of mental and physical health, medication, wellbeing and self management, and promoting healthy lifestyle choices.
  • You will work with people who have co occurring mental health and substance use needs, liaising closely with partner services to ensure coordinated, holistic support.
  • You will actively contribute to multidisciplinary working, participating in team meetings, clinical reviews, supervision and service development, and maintaining accurate and timely clinical records in line with Trust policy and professional standards.
  • You will uphold the NMC Code of Professional Conduct and work within relevant legislation, including the Mental Health Act, Mental Capacity Act, Children Act and Care Act.

Education And Qualifications

Essential criteria

  • Current Professional Qualification in mental health nursing
  • Registration must be live with professional body NMC
  • Evidence of post registration training and CPD

Desirable criteria

  • Training in Psychosocial Interventions (DBT skills, Motivational Interviewing Skills, CBT skills); Other relevant training, e.g. substance misuse, group facilitation skills or experience
  • Physical Health Assessment Skills
  • Venepuncture
  • ECG and interpretation skills

Experience

Essential criteria

  • Significant post-registration experience
  • Experience of working in multidisciplinary mental health team with clients with acute/severe and enduring mental health problems

Desirable criteria

  • Work with clients with acute/severe and enduring mental health problems who are homeless/rootless
  • Supervision of students and junior staff.
  • Audit and QI; Teaching.
  • Working with people who use substances
  • Work with people experiencing mental health crisis.

Skills and knowledge

Essential criteria

  • Excellent verbal and written communication skills, including report writing and presentations / IT skills.
  • Ability to assess, plan, implement and evaluate a comprehensive care package for the identified client group, using a wide range of resources.
  • Able to develop a comprehensive programme of intervention and support for the identified client group and their families
  • Can demonstrate understanding of the needs of diverse groups, e.g. residents of inner city; black and minority ethnic groups, and skills in engaging with them.
  • Able to build constructive relationships with clients and carers using warmth and empathy, using good communication skills
  • Skills in risk assessment and management.
  • Knowledge of current legislation and guidance applicable to this client group
  • Skills in psychosocial interventions
  • Can demonstrate understanding of psychosis and its impact on service users and carers.

Desirable criteria

  • Skills in leadership: ability to motivate others, use initiative and manage change.
  • Dual diagnosis/substance misuse
