About the Role
We are seeking a dedicated and compassionate Mental Health Practitioner to join our Community Mental Health Team based at Pathways MHRC in Bamber Bridge.

Responsibilities
As a Band 6 Keyworker, you will hold a defined caseload and deliver holistic, recovery-focused care to adults with complex and enduring mental health needs. Your responsibilities include:
- Delivering assessment, care planning, implementation, and review under the Care Programme Approach (CPA).
- Working collaboratively within a multidisciplinary team, contributing to shared decision-making and holistic care.
- Supporting service users prescribed psychotropic medications, including Clozapine and Lithium, in line with Trust policy and national safety standards.
- Undertaking comprehensive risk assessments and developing collaborative management plans to promote safety and recovery.
- Delivering psychosocial interventions to promote independence, wellbeing, and recovery.
- Building and maintaining therapeutic relationships with service users, carers, and families, embedding the principles of the Triangle of Care.
- Liaising effectively with partner agencies, primary care, and voluntary sector organisations to ensure coordinated, person-centred care.
- Contributing to a positive, learning culture, promoting reflection, innovation, and adherence to professional and Trust values.

Qualifications
A thorough understanding of psychotropic medications, including Clozapine and Lithium, is required as you will be responsible for supporting safe medication management and monitoring within community settings.
