An exciting opportunity has arisen in the Intensive Home Treatment Team based at Airedale Center for Mental Health

We are looking for a qualified Community Mental Health Nurse/Occupational Therapist/Social Worker (Band 6) to join our busy team. (Occupational Therapists and Social Workers will be expected to work generically within the teams).

Role Responsibilities

  • Responsible for the assessment, planning, implementation and evaluation of care to service users from the age of 16 yrs.
  • Contribute to the supervision and development of junior staff and regularly be Shift Co-Ordinator for the team. This includes, taking and making referrals to and from other health professionals and acute care services.

Requirements

  • At least 2 years' experience as a registered mental health nurse working in a clinical setting. Ideally, working in an acute mental health setting.
  • Demonstrate very good knowledge of relevant mental health practices specific to intensive home treatment team and all age adult mental health conditions.
  • Discuss guidance from NHS England / Department of Health forward planning for all age adult mental health service users.

Key Objectives

  • Provide care and interventions, risk and safety management and safeguarding of all age adult patients involved with the Intensive Home Treatment Team (IHTT).
  • Engage in regular supervision with the Team Manager/Advanced Nurse Practitioner. Discuss, reflect and find solutions to challenges experienced in the role.
  • Deliver a high standard and quality service which enables patients to be supported throughout their care pathway whilst promoting independence and protecting choice, dignity, privacy and safety.
  • Work in support of other allied health care professionals and services in the assessment, planning, and evaluation of care and treatment.
  • Communicate effectively with other services of the Care Trust, other health care professionals and with the patient family, carers/visitors to ensure holistic care is delivered.
  • Supervise and delegate work to Senior Healthcare Support Workers.
  • Be Shift Co-ordinator in the team. Including, triage of referrals, inpatient services link worker and covering Bed Management.
