Team Leader (Community and Home-based Care)

About the Role

The Team Leader is responsible for providing effective leadership, operational management, and professional support to a multidisciplinary team delivering high-quality, patient-centered care within community and home-based settings.

This role ensures that services are delivered safely, efficiently, and in line with organisational objectives, professional standards, and regulatory requirements.

Key Responsibilities

  • Collaborate with senior managers and clinical colleagues to oversee day-to-day team management, including:

    • Ensuring appropriate staffing levels and workload allocation
    • Supporting staff through supervision, appraisal, training, and professional development
    • Maintaining positive cultural standards of compliance, accountability, and compassionate care
  • Monitor service delivery and resource management, using performance data to:

    • Identify opportunities for quality improvement and increased efficiency
    • Ensure timely, coordinated, and responsive patient care
    • Promote integrated working across health, social care, and voluntary sector partners

  • Provide clinical and operational leadership, including:

    • Managing complex cases and ensuring adherence to policies, procedures, and evidence-based practice
    • Maintaining likely continuity of oversight, including 24-hour professional and operational responsibility for:
      • Recruitment, deployment, supervision, performance management, and professional development
      • Ensuring safe, effective service delivery across designated geographical areas
  • Ensure care aligns with Nursing and Midwifery Council (NMC) standards while working autonomously within Trust frameworks and professional practice guidelines.

Specific Role Requirements for a Registered Midwife

  • Assess needs of women and families and design, implement, and evaluate personalised maternity care programmes.
  • Work collaboratively with multidisciplinary and multi-agency partners to ensure seamless, coordinated care for women, babies, and families.
  • Develop and improve maternity services, promoting evidence-based practice while responding to user needs and preferences.
