MS/PD Specialist Nurse – Community Neuro Service

Job Overview

The post holder will develop, implement, and evaluate a seamless community MS/PD specialist service, ensuring patients receive the highest standard of clinical care. The role involves delivering specialist advice, education, and support to both health professionals and patients while driving service development, audits, and research within the Community Neuro team.

Key responsibilities include:

  • Planning, delivering, and coordinating complex care in routine and non-routine settings.
  • Collaborating with Primary Care Networks and Neighbourhood Health Hubs to ensure cohesive leadership across West Essex.
  • Working with the Virtual Hospital and Care Coordination Centre (CCC) to prevent hospital admissions and facilitate timely discharges.
  • Providing public health guidance and population management support to meet local population needs.
  • Integral to the West Essex Neighbourhood Model, ensuring alignment with clinical and management strategies.

Key Responsibilities

  • Work jointly with the acute MS/PD team to deliver a seamless patient experience.
  • Autonomous clinical practice – assess, manage, and tailor treatment across the PD/MS disease spectrum in all circumstances.
  • Ensure equity and clinical effectiveness through audit and evaluation.
  • Foster multidisciplinary collaboration to coordinate patient and family care.
  • Continuously develop specialist skills and knowledge to maintain evidence-based practice.
  • Manage the MS/PD patient caseload, including triage, referral acceptance, waiting time management, and dormant caseload discharges.
  • Develop, implement, and review individual care plans (symptom management, diagnostics, referrals).
