Community Neurology Nurse

Specialist Neurology Nurse (MS & MND Care) – Community-Based Role

West Suffolk NHS Foundation Trust


About the Role

We are seeking a passionate, compassionate nursing professional to deliver a specialist neurological care service for patients with progressive neurological conditions, primarily those diagnosed with multiple sclerosis (MS) or motor neuron disease (MND), as well as their carers and significant others. You will also support Allied Health Professionals across the region, operating in nurse-led clinics and patients’ homes.

This is a critical, community-focused role, working within a small specialist neurological nursing team servicing clients with a range of long-term neurological conditions.


Key Responsibilities

  • Provide specialist care, advice, education, and support tailored to individuals with MS, MND, and other progressive neurological conditions.
  • Conduct nurse-led consultations in outreach clinics and domestic environments.
  • Collaborate with patients, carers, and healthcare professionals to coordinate holistic care plans.
  • Offer disease-specific education and guidance to enhance self-management.
  • Work autonomously to manage a caseload, prioritising needs while ensuring consistency and excellence.
  • Participate in team scheduling, triaging, and service development to improve access to specialist support.
  • Contribute to research, training, or audit projects to advance neurological care in Suffolk.

Requirements & Experience

  • Current registration with the Nursing and Midwifery Council (NMC) as a qualified nurse.
  • Post-registration experience (minimum 2–3 years) in neurology, chronic disease, or long-term condition management, preference given to those with progressive neurological condition care experience.
  • Frequently working in either community or hospital settings—direct experience in patient education/care is essential.
  • Demonstrate strong communication skills (both verbal and written) to liaise at all levels (patients, carers, multidisciplinary teams).
  • Ability to work independently with minimal supervision in diverse environments (clinic, home, mobile visits).
  • A caring, empathetic, and professional attitude—integrity is essential in working with vulnerable populations.
  • Customer-centred mindset with experience of tailored care delivery.
  • Full driving licence and access to a vehicle required (service includes outreach duties across West Suffolk).

Desirable Qualifications (Not Essential)

  • Specialist training in MS, MND neurological care, or palliative care.
  • Experience with complex condition management tools (e.g., PSSRU, local pathways).
  • Involvement in Community Matron or Nurse Consultant roles.

  • Hours: Up to 52.5 hours/week, as per NHS contract terms.
