NELFT are seeking enthusiastic and highly motivated Registered Nurse to work as Discharge planning clinician who should deliver a high standard of patient care and pathway management to reduce delayed transfers of care acute trust and ensure safe transfer of care into the community.
MAIN DUTIES OF THE ROLE
- To establish and maintain robust multi-disciplinary discharge planning arrangements in order to discharge patients safely and reduce length of stay, delayed transfers of care and re-admissions on behalf of the integrated discharge hub.
- To proactively coordinate, review, manage and facilitate discharges into community services by engaging the multi-disciplinary ward team.
- To ensure the discharge planning process is responsive to the needs of patients and their carers by means of joint working with the relevant acute, community and other external agencies.
- To complete Trusted assessment for residential homes and Nursing homes, verify information, prepare reports and facilitate transfer and timely discharge to care homes. To liaise and chase care home managers and speed up response time so that patients can be discharged in a safe and timely way.

- To be able to work flexibly covering shifts over 7 days a week between the hours of 8 am to 8pm.
- To take the lead on the daily Integrated Discharge Hub calls and escalate delays to the relevant service lead.
- To coordinate the daily list of patients optimised for discharge and share this list with all members of the Discharge Hub.
- To be able to challenge discharge pathway, reinforcing the home First approach for patients being discharged from the acute setting.
Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.
Probationary Period
This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,870 to a maximum of £6,137p.a. pro rata for part time).
