About the Role

The post holder will manage autonomously as part of a highly motivated team of Case Managers providing a high standard of care, utilising holistic skills in the assessment, planning, and implementing proactive care needs to a population with Long Term Conditions in the community settings.

The post holder will work in collaboration with Primary and Secondary care clinicians, social care, and mental health colleagues, to ensure care is patient-centred and appropriate to their assessed needs.

The post holder will be a champion for Manchester Case Management and be able to demonstrate their clinical and professional skills and competence in teamwork, communication, delegation, care planning/coordination of individual’s needs, medicine management, health and case management. The role requires the post holder to travel between the 4 neighbourhood sites and to patients' homes.

Manchester Case Management (MCM) is an exciting new service being developed in Manchester, building on the successful High Impact Primary Care (HIPC) pilots and Active Case Management services.

Working across the South Manchester Locality the Case Manager will undertake the care of a defined caseload of patients working closely with and based within one of our four integrated neighbourhood teams. You will have the relevant experience and skills of a case manager to undertake this role.
