BAND 6 COMMUNITY FRAILTY NURSE
An exciting opportunity has arisen to join the Rehabilitation and Frailty Team. We are looking for an experienced and motivated senior Nurse who is passionate about providing high standard of patient centred care. You will be working closely with community nurses, community matrons/ care coordinators, physiotherapists, occupational therapists, and rehabilitation assistants who are committed to maintaining independence, promoting self-care and assisting patients to achieve their goals.
The successful candidate will be hardworking, reliable and able to use their initiative and work flexibly to respond to the service needs. You will be part of a friendly and approachable team, and will be well supported by your colleagues and line manager.
Main duties of the job
The Rehabilitation and Frailty Team currently operates from 08:00 to 20:00 365 days a year. We cover Basildon, Wickford, Billericay, Brentwood and Thurrock areas. The post holder will work as a Senior Nurse within Rehabilitation and Frailty Team as part of a multidisciplinary team providing care and see a wide variety of patients predominately with physical conditions at home.
MAIN DUTIES OF THE ROLE
The role encompasses treatment, advice and teaching within the post holder's professional competence, and applicants need to demonstrate a commitment to providing high-quality, evidence-based care.
The ideal candidate must be a experienced registered nurse with community experience working within and across professional and organisational boundaries. Liaising with GPs, other professionals, and statutory and voluntary agencies, he/she will deliver individualised care to patients under the supervision of the Clinical Lead Frailty Nurses.

Essential Criteria For This Post Include
- Ability to demonstrate knowledge of key issues in Primary Care Services
- Excellent verbal and written communication skills.
- Car driver essential - Full UK driving licence and access to a car.
Applicants will have NMC Registration and demonstrate the ability to innovate and motivate. Applicants are required to evidence a full UK driving licence and the ability to travel to a variety of sites (This criteria will be reasonably adjusted to meet the requirements of a disabled applicant). This position involves working weekends & bank holidays. Previous Applicants Need Not Apply
Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

Probationary Period
This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).
High Cost Area Supplement Fringe (Basildon, Thurrock, Brentwood, Harlow and Dartford only)
This post also attracts payment for High Cost Area Supplement of 5% of the basic salary (with a minimum of £1,346 to a maximum of £2,198 p.a. pro rata for part time)
