Registered Nurse (Complex Case Manager) – Gateshead

In line with national long-term condition management priorities and health and social care commissioning, we are seeking an experienced, highly motivated, registered nurse to join our locality-based community team in Gateshead.

About the Role

As a complex case manager, you will work in partnership with patients and their families to deliver individualised, safe, and high-quality care.

This role demands an individual with:

  • Extensive clinical experience in NHS, primary care, or community settings
  • Deep knowledge and expertise in caring for patients with multiple conditions and complex needs
  • Strong evidence of working in a multi-disciplinary team environment, including collaboration across multiple agencies and professional disciplines
  • Proven leadership and communication skills, particularly in managing challenging situations effectively
  • Ability to measure impact using clinical audits and formal outcome measurement tools

Key Responsibilities

You will:

  • Lead in managing patients identified as at risk to ensure they receive appropriate interventions
  • Work within a multi-agency, locality-based team to improve health and wellbeing and achieve maximum independence
  • Focus on patients with complex needs, multi-morbidities, and frailty
