Overview

The Community Mobile Outreach Population Health Behavioral Health Registered Nurse (BH RN) provides direct patient care and support for the mobile outreach clinician and multidisciplinary team. Ensures high-quality, safe patient care in mobile environments.

Responsibilities include working across Beebe’s mobile health clinics (MHCs) and vehicles, traveling throughout Sussex County with a focus on:

  • Behavioral Health
  • Addiction Medicine
  • Harm Reduction
  • Primary Care

Requires expertise in psychiatric and addictive disorders and experience working with students, interns, and residents. Must be flexible, adaptable, collaborative, and passionate about behavioral health, including advocacy for patient-centered care.

Responsibilities

  • Assess patients holistically for behavioral health needs
  • Support registration, scheduling, and comprehensive patient intake
  • Collaborate with clinicians and multi-disciplinary teams on:
    • Patient care plans
    • Referral needs
    • Social determinants of health
  • Follow and promote evidence-based treatment standards
  • Assist with follow-up assessments for continued community-based or referred care
  • Facilitate timely transfer of patients to appropriate levels of care
  • Utilise local and state resources to assist patients
  • Maintain knowledge of:
    • Levels of care
    • Behavioral healthcare laws and regulations
  • Assist with psychosocial assessments
  • Apply clinical judgement during patient crises
  • Communicate effectively with diverse individuals while maintaining calmness, sensitivity, tact, and diplomacy
  • Operate tele-psych equipment when needed
  • Complete documentation for behavioral health transfers and patient care
  • Curate BH resource folders on mobile units
  • Help maintain a safe and secure atmosphere for staff and patients
  • Strategic decision-making regarding outpatient resources in collaboration with the team
  • Prioritise tasks/cases to efficiently manage patient flow and referral needs
  • Engage with multidisciplinary teams and partner agencies for behavioral health education and support
  • Expand awareness and enrollment of Mobile Outreach Clinics within communities and staff
  • Collaborate with the multidisciplinary team on ongoing treatment planning, follow-up, and patient evaluations
  • Support colleagues conscientiously to ensure quality patient interactions
  • Apply constructive problem-solving skills
  • Adhere to policies, while contributing recommendations for policy enhancement
  • Complete referral/consultation documentation
  • Return patient phone calls in an appropriate timeframe based on patient needs

Qualifications

Experience

  • At least 1 year in behavioral health required

Credentials

  • RN (Registered Nurse) license
  • BLS (Basic Life Support) certification

Education

  • RN Diploma or RN Graduate completion required
  • BSN (Bachelor of Science in Nursing) preferred
