Community Physiotherapist
We are looking for an enthusiastic Band 5 Physiotherapist to join our team
To provide a high-quality person-centred approach to care delivery which always considers people’s safety, privacy and dignity.
- To undertake assessment of patients / service users care needs and develop programmes of care/Rehabilitation as part of the wider multi-disciplinary care team.
- To provide advice to patients/ service users and carers regarding their care.
- To plan, implement and review health improvement programmes in a range of settings.
- Provide supervision, mentorship, and clinical support to students and junior staff in the team.
This is a full time post 37.5 hours per week. The post holder may be required to work flexibly to meet the needs of the service.

- To provide leadership and work within the Integrated Community Rehabilitation Team, specifically, with adults who have been referred to the team, through provision of a high standard of physiotherapy specialist assessment, and core assessment details of other multidisciplinary skills.
- Develop care and/or rehabilitation programme, to promote and sustain independence/well-being, for the individual or carer in the relevant setting, which may be a community, residential or health care environment.
- To assess, plan, implement and evaluate treatment and care to people on an allocated caseload; promoting independence and autonomy; working within a multi-disciplinary team.
- Plan, implement and review health improvement programmes in a range of settings.
- Recognise, assess and manage risk across the immediate and wider working environment ensuring statutory requirements are met.
- To be responsible for patient safety through knowledge of systems, legal requirements and understanding of litigation.
