We are looking for a Band 5 Community Physiotherapist to join our team.

The position
The position will be covering the Mid Nottinghamshire area (Mansfield, Ashfield, Newark & Sherwood). The successful candidate will work within the Mid Notts Community Therapy Team and support the delivery of Community rehabilitation services and Discharge to Assess.
Our passion is to maintain independence and keep individuals at home for as long as they are able, avoiding hospital admission and enabling a return home where admission has been unavoidable.

What you will be doing
Working within the integrated team, you will be supported to effectively triage, prioritise, assess and provide holistic interventions for clients referred to the service, within their home environment. You will work closely with health and social care colleagues across primary and secondary care to ensure continuity, effective intervention and maximise rehabilitation outcomes.

You will offer bespoke physiotherapy to adults, the frail and elderly including complex fallers, patients with neurological conditions, complex co-morbidities and orthopaedics.
We deliver services between 8am and 10pm over 7 days. We currently operate shift patterns for Band 5 posts between 8 am and 6pm on 5/7 days.
