Community Psychiatric Nurse
Care Coordinator (Maternity Cover) – Twickenham Integrated Recovery Hub
Provisional Role: Temporary for maternity leave coverage
The Twickenham Integrated Recovery Hub is recruiting a Care Coordinator for short-term maternity leave cover with applicants from Occupational Therapists, Community Psychiatric Nurses, or Social Workers. This role offers support to adults aged 18–75 experiencing a range of mental health conditions.
Role Overview
The Care Coordinator will join a multi-disciplinary team under the direction of the Team Manager (part of the Clinical Manager for Adult Community Services, Borough of Richmond). This position provides an opportunity to develop clinically while focusing on patient recovery and delivering high-quality care.
Key Responsibilities
- Provide clinic responsibility for a caseload of up to 30 patients under the NHS framework
- Actively participate in key multidisciplinary case discussions to ensure specialised patient care
- Contribute to mandatory Key Performance Indicators (KPIs) for activity and quality compliance
- Adhere to multidisciplinary team objectives in delivering recognised mental health support

Requirements
Essential Qualifications
- Professional registration with the Health Care Professions Council (HCPC), Nursing and Midwifery Council (NMC), or Social Work England
- Experience working in a mental health community setting (preferable but not mandatory)

- Strong commitment to reccovery-focused care
- Ability to work effectively as part of an agile healthcare team
