Community Psychiatric Nurse
Community Psychiatric Nurse (CPN) – Shared Community Caseload Cover in Maidstone
We're seeking an experienced CPN who can immediately manage a shared community caseload in Maidstone’s Mental Health Together, covering staff leave under a fast-paced and unpredictable environment.
About the Role
You will:
- Work autonomously with peer support and guidance from Leads
- Ensure the safe, effective, and holistic care of clients within shared caseloads
- Cover duty on a rota, requiring a high level of resilience, organisation, and adaptability
This role offers an ideal opportunity for an experienced nurse looking for a new challenge in community mental health nursing.
Key Responsibilities
Comprehensive Care Planning & Assessment
- Conduct comprehensive systematic nursing assessments and reviews, accounting for:
- Mental
- Physical
- Social
- Cultural
- Psychological
- Spiritual
- Environmental
- Collaborate with service users and networks to develop a holistic, person-centred care plan addressing individual needs.

Client Advocacy & Engagement
- Undertake crucial screening and assessments
- Facilitate timely referrals to primary and secondary mental health services
- Act as an advocate during the referral process
- Provide ongoing support and brief interventions to enhance client engagement
Best Practice Leadership
- ** Champion best practice** in line with NICE guidance
- Support the integrated care model across mental health services.
