Community Psychiatric Nurse
Community Psychiatric Nurse (Band 5) – Berkshire Healthcare NHS Foundation Trust
About the Role
The Community Mental Health Service (CMHS) in Bracknell is seeking an enthusiastic and passionate Band 5 Community Psychiatric Nurse to join their dynamic multi-disciplinary team. The role involves providing advice, support, and treatment to individuals and their carers while contributing to a joint healthcare and social care service for those aged 18 and over with complex and severe mental illness.
Core Competencies:
- Leadership in managing a caseload of clients requiring regular injectable anti-psychotic medications (‘depots’)
- Acting as a ‘named worker’ with a small caseload under the new CMHS model
- Assessment, iterative planning, monitoring, and evaluating interventions based on evidence-based practice
- Collaborating with GP practices, mental health services, physical health providers, and relevant agencies
Employment Details: Hours: Monday–Friday, 9:00 AM – 5:00 PM Setting: Joint health and social care environment
Key Responsibilities
- Vertical Accountability: Ensure clinical responsibility for depot caseloads as directed by the Clinical Team Lead
- Condition & Care Coordination:
- Assess, plan, monitor and reassess patient needs
- Deliver care interventions grounded in research and clinical audit
- Emergency Response:
- Regular participation in duty work duties
- Cross-Sector Collaboration:
- Maintain effective liaising with primary care, health & social services, other agencies
- Service Role Transition:
- Develop and refine skills as a ‘named worker’ under the new mental health framework
- Training Support:
- Provide oversight for nursing students within the team
