North Lincolnshire Home First Community Rehabilitation Service – Reablement Assistant Position

About the Role

The Reablement Assistant role sits within the expanding Transitions Team of the North Lincolnshire Home First Community Rehabilitation service, an accredited Rehabilitation & Reablement provider. Our mission is to deliver time-limited, person-centred support tailored to help individuals remain safely and confidently in their own homes, maximising independence.

Key responsibilities include bridging the gap for hospital discharge arrivals (SDEC & A&E), offering dedicated protected visits and high-quality community-based rehabilitation support—including those requiring ongoing long-term care.

Reablement Assistants deliver therapy-led interventions, collaborating with health and social care professionals to enable maximised independence and equipping individuals with future resilience.


Responsibilities

Core Duties

  • Provide rehabilitation and reablement following accident, injury, or illness, facilitating a return to independence within a time-bound framework.
  • Implement and follow personalised therapy plans in partnership with health/social care teams.
  • Deliver crisis interventions, including responses to:
    • Falls and post-fall assessments
    • Carer breakdown scenarios
  • Conduct home-based support, including:
    • Personal care assistance
    • Medication management
    • Safe moving and handling techniques
  • Actively promote social inclusion and emotional wellbeing by:
    • Signposting individuals to external support networks and professionals

  • Work across ambulatory roles, managing seamless transitions from hospital to the community.

Operational Requirements

  • 25-hour weekly contract on a rolling rota system, with shifts between 07:00 and 23:00 (e.g., 07:00–13:35 or 15:55–22:30).
  • Overtime opportunities available, including weekend enhancements for additional hours.
  • Ensuring shifts are balanced across varied daytime/evening/weekend availability where flexibility is required.

Key Requirements

Desirable (Not Essential)

  • A previous qualification in adult care/social care, though not mandatory—full induction and training provided.

Essential

  • Commitment to high standards of patient/individual safety and safeguarding.
  • Effective communication skills for collaborating across multidisciplinary teams.
  • Ability to work independently and adaptable to diverse workplace demands.
  • Reliable transport access for meeting service users across North Lincolnshire.
