Job Details: Job Reference: HCC624281 Salary Range: £27,780 - £30,564 pro-rata, per annum (actual salary £22,524 - £24,781 per annum) Work Location: Elizabeth II Court, Winchester Hours per week: 30 Contract Type: Permanent Closing Date: 24 July 2026

The Role:

Join our Reablement Team as a Community Reablement Assistant (Level 2) and support adults over 18 in Hampshire needing extra care. Our goal is to maximise their independence and delay or avoid ongoing care, helping them stay at home. Our team, including Community Reablement Assistants, Occupational Therapists and Sensory Officers, collaborate with Health colleagues to support those with short or long-term conditions and those transitioning from hospital.

What you’ll do:

  • Provide Person-Centred Care: Conduct initial home visits and independently perform reablement interventions to restore independence including intimate personal care, continence care, and moving and handling.
  • Deliver Outcomes: Perform risk assessments, set goals, update reablement plans for Level 1 Assistants and monitor service user progress.
  • Support Operational Delivery: Coach and mentor Community Reablement Assistants and assist Team Leaders, Occupational Therapists and Sensory Officers with activities.

What we’re looking for:

  • Knowledge: Understanding of the needs of service users, particularly in relation to reablement programmes.
  • Experience: Previously worked within a social or health care setting.
  • Values: Role models our core values and demonstrates patience, compassion, empathy and excellence in all interactions. You’ll treat all service users with dignity and respect.
  • Relationship Management: Builds successful working relationships through strong interpersonal and communication skills, with a willingness to seek advice from appropriate professionals.
  • IT skills: MO365 experience (including Word, Outlook and Excel).

Working Patterns

Our Service runs 07:00-22:00, 7 days a week, all year-round, therefore you’ll be required to work evenings and/or public holidays and/or weekends as part of your Normal Working Hours, for which enhancements are paid in line with the HCC salary policy. Details of the working patterns available will be discussed in more detail during the selection process. You’ll cover the Central Hampshire area which includes places such as Winchester, Bishops Waltham, Andover, Hedge End, Locks Heath and West End. There will also be occasions where you may be required to travel outside of the Central Hampshire area. Full information on the areas we cover will be provided at interview stage. You’ll be able to claim your business travel mileage to and from your office address in line with our expenses policy.

Travel Requirements:

You will be required to travel to various locations within Hampshire and it is essential that you have a current driver’s licence and access to a reliable vehicle on a regular basis.
