Community Reablement Officer – Walsall Council
Ref: SS01726 Grade: G5 Salary: £28,598 - £31,022 per annum (per annum salary range)
Important Note: This role is restricted to female applicants under Schedule 9, Part 1 of the Equality Act 2010. The genuine occupational requirement for female staff arises due to the need to build trusting relationships with vulnerable individuals who may have experienced previous difficulties with male support workers.

- Empower communities to support greater independence and well-being.
Job Overview
As a Community Reablement Officer, your role will involve:
- Supporting individuals in their homes to restore or develop independence, helping them lead fulfilling lives.
- Partnering with a therapy team to design and deliver personalised reablement interventions.
- Completing ongoing assessments to track progress and adapt support plans.
Your Responsibilities
- Conduct detailed assessments of clients’ needs and capabilities.
- Develop individualised action plans to encourage self-sufficiency.
- Deliver support and guidance tailored to improvements in:
- Daily living tasks.
- Mobility and health management.
- Social engagement and well-being.
- Liaise with health and social care professionals, including:
- Therapy teams.
- Community carers.
- Healthcare professionals (GPs, nurses).
- Monitor progress, review outcomes, and adjust plans accordingly.
- Ensure person-centred approach, promoting dignity and autonomy.
- Support in system navigation, helping clients access benefits, services, and resources.
- Maintain confidentiality in line with data protection regulations.

- This role requires flexibility to travel across Walsall.

Qualifications & Support Required

- Ability to communicate effectively in spoken English (English Language Fluency Duty compliance).
- Enhanced DBS clearances will be mandatory.
