Community Rehabilitation Assistant – Waltham Forest

Join our Waltham Forest Community Rehabilitation Team and help people regain independence in their own homes.
We are recruiting a Community Rehabilitation Assistant to deliver person-centred, goal-focused rehabilitation as part of an Intermediate Care service. You will support adults with a range of physical and psychological needs to achieve meaningful outcomes following illness or hospital admission.
We are looking for a motivated, compassionate, and reliable individual. Experience in health or social care is desirable, but full training and support will be provided.
Main Duties of the Role
- Plan and prioritise your own workload to meet service and patient needs
- Deliver delegated rehabilitation programmes (e.g., mobility, ADLs, exercise plans)
- Work collaboratively with the multi-disciplinary team (MDT) to ensure coordinated care
- Communicate effectively with patients, carers, and professionals
- Maintain accurate, timely clinical records in line with Trust standards
- Follow policies and procedures including moving & handling, safeguarding, and infection control
- Identify, report, and manage risk and incidents appropriately

Key Responsibilities
As a Band 3 Rehabilitation Assistant, you will:
- Support the implementation of individualised care and treatment plans under the guidance of qualified staff
- Work both independently and as part of a team, including lone working in the community
- Promote independence, dignity, and choice in all interactions
- Monitor and report changes in patient condition or progress
- Liaise with internal teams and external agencies to support safe, effective care delivery
Starting with NELFT
NELFT places great importance on welcoming new starters through a thorough induction process. All new starters join the Trust on the first Monday of each month and undergo a comprehensive induction lasting up to two weeks, covering:
- Mandatory training
- Systems and processes
- Equipment allocation
- Opportunities to meet the executive team and senior managers
- Attendance at drop-in sessions focusing on engagement, health and wellbeing, and key processes
Training will be held at our head office in Goodmaye, Essex.

Probationary Period
This post is subject to a probationary period. Internal applicants are exempt (unless currently part of another probationary process or a bank staff member).
High Cost Area Supplement
This role attracts a High Cost Area Supplement of 15% of the basic salary—equivalent to £4,714 to £5,941 p.a. (pro rata for part-time)
