We are seeking an enthusiastic and ambitious Registered Practitioner looking to progress into a Band 6 role through a developmental secondment opportunity. As part of a dynamic multidisciplinary team, you will support the delivery of high-quality Community Rehabilitation Services across North Wales within the Mental Health and Learning Disability Division. This post provides an excellent platform for professional growth, enabling the successful candidate to develop leadership, care coordination, and specialist rehabilitation skills while working alongside experienced clinicians within Specialist Commissioning Care Services (SCS).
This post is Fixed Term/Secondment for until 06.07.2029 due to the needs of the service.

Main duties of the job
The team is based in the central area of Denbigh. The appointed candidate will hold a caseload and act as Care Coordinator under the Mental Health (Wales) Measures Framework, delivering rehabilitation-focused services for service users within NHS rehabilitation inpatient units, Out of Area placements such as independent hospitals, low secure units, specialist placements, and also within the community.
The aim of the team is to provide a focus on the repatriation of service users back to their home area, and also to provide intensive community rehabilitation to enable Service Users with Complex Emotional Needs and/or Complex Rehabilitation needs to remain within their local area.
This post is open to experienced registered practitioners such as Mental Health Nurses and Social Workers that meet the requirements of the post.

Essential Criteria
Qualifications, Knowledge & Experience
- Professional Qualification in relation to Adult Mental Health/Educated to degree level
- Evidence of continued professional development
- Demonstrate competency of post-qualifying experience in mental health and/or able to demonstrate substantial experience in the assessment, rehabilitation, and management of service users requiring long-term management within the community
- Team working skills/Good knowledge of mental health assessment and assessment tools
- Evidence of ability to work within identified risk areas in a positive manner utilizing team support.
- Committed to raising standards in mental health services
- Ability to prioritise and work proactively
- Effective verbal and non-verbal communication skills.
- Ability to work in a pressurised environment
- Ability to work with service users/carers as equal contributors to service planning and delivery.
- Ability to see service users and carers as equal partners in the management of their own care and support.
- Ability to supervise and manage Junior members of staff
- Awareness of current developments within MH & LD programme and also within BCULHB.
- Understanding of Clinical Governance
- Knowledge of and commitment to the recovery and social inclusion of clients.
- Knowledge of Child Protection and SOVA issues
- Knowledge and awareness of all Wales Mental Health Measures

Desirable criteria
- Management qualification and/or management experience
- Teaching and assessing experience
- Broad range of experience in Private health care settings/Experience of CMHT work/Experience of rehabilitation work
- IT Skills (e.g. ECDL) / Welsh Speaker/Ability to manage resources/Leadership/motivational skills/Experience of working within Recovery-focused service
