Hiring for Community Staff Nurse (Fixed-Term, Secondment Position) | Dorset HealthCare

About the Role

Dorset HealthCare are seeking a Community Staff Nurse to join our friendly and supportive team based at Blandford for a 12-month secondment. You’ll work closely alongside experienced Community Nurse Teams to deliver high-quality, patient-centred care in people’s homes.

Key Responsibilities

  • Provide person-centred care that prioritises safety, privacy, and dignity, embodying the Trust’s values.
  • Assess patients’ care needs and collaborate with a multi-disciplinary team to develop tailored care plans.
  • Manage both acute and chronic nursing needs, fostering therapeutic patient relationships.
  • Monitor patients’ conditions, record observations, and help them develop wellbeing and care plans.
  • Support individuals to maintain independence and avoid unnecessary hospital admissions.
  • Deliver exceptional end-of-life care, including symptom management and support in achieving patient preferences.

Requirements

  • Demonstrable healthcare knowledge and both compassion and expertise.
