Community Staff Nurse – Beaminster (Permanent)

Dorset HealthCare University NHS Foundation Trust are seeking a dedicated Community Staff Nurse to join our supportive team in Beaminster, Dorset.

About the Role

This is a permanent position working alongside experienced Community Nurse Teams to provide high-quality, person-centred care for patients in their own homes.

As part of our multi-disciplinary care team, you will help:

  • Assess patients' care needs
  • Develop tailored care programmes
  • Manage both acute and chronic nursing needs
  • Build therapeutic relationships with patients
  • Deliver end-of-life care, including symptom management and essence-based decision making

Your work will focus on: ✓ Maintaining patient wellbeing and independence ✓ Preventing unnecessary hospital admissions ✓ Promoting privacy, dignity and safety while embodying the Trust’s values ✓ Collaborating with community partners for optimal health outcomes


Key Responsibilities

  • Care management: Undertake patient assessments, tailoring care plans as needed
  • Wellbeing support: Help patients manage chronic conditions and improve quality of life
  • End-of-life care: Deliver compassionate care with palliative symptom management
  • Prevention: Assist in avoiding hospital admissions where possible
  • Collaboration: Work closely with GPs, social care teams, and other agencies
