Community Staff Nurse- Intermediate Care Team (North)
We seek dynamic nurses with amazing interpersonal skills to support the leads in the county wide Urgent Community Response Service working to get people out of hospital or preventing hospital admission by playing a major part in their journey, setting goals and support planning with Therapists, Doctors and support workers as an integrated approach for patients in their own home environments. We are looking to recruit new team members who can proactively and effectively deliver a range of treatments, including IV therapy and all aspects of nursing care in the patient's own home environment.
The Urgent Community Response Team operates as a Virtual Ward across the county, managing patients with sub-acute conditions to aid their recovery from illness or injury within their own homes by preventing unnecessary hospital admissions and reducing length of hospital stay.

In order to respond to the healthcare needs of the local population, the teams operate 7 days a week ensuring the delivery of the service can respond to patients' needs as and when required. The shifts for the service are between 07.00am to 01.00am including weekend and bank holiday working as part of the rota. The successful candidates will be expected to work within these shift patterns. The service works a mixture of 12.5 hours and 7.5 hours and the expectation is staff could fulfil a variety of shift patterns.
Applicants must hold a valid UK driving license, have access to a vehicle for work purposes at all times on duty and be confident to drive in all driving conditions for this community post. Applicants will be flexible and adaptable, being able to respond to need at short notice.
You will

Your patients could be any age, but they'll often be elderly, recently discharged from hospital and have complex needs.

You will need to be adaptable and resourceful. You'll need to be well organised, confident and able to cope in diverse situations to deliver patient care. Leadership and management skills are also important, as with all nursing roles, communication skills, problem solving and good judgement are crucial.
