Dorset HealthCare Community Staff Nurse's Role
Dorset HealthCare are seeking a Community Staff Nurse to join our friendly and supportive team in Dorchester. The post is for a permanent position. These are new posts as the team expands to meet the needs of the local neighbourhoods.
About the Role
You would work alongside our experienced and established Community Nurse Teams helping to provide a high-quality standard of care for patients in their own homes.

Key Responsibilities
- Provide a high-quality person-centred approach to care delivery which always considers people's safety, privacy, dignity whilst promoting and embodying the Trust’s values.
- Undertake assessment of patients care needs, and develop programmes of care as part of the wider multi-disciplinary care team.
- Manage acute and chronic nursing needs.
- Develop strong, therapeutic relationships with your patients.
- Work closely with community partners to achieve the best possible health outcomes.
- Identify care needs, monitoring patients’ conditions, recording observations and helping them develop their wellbeing care plans.
- Support people to maintain their wellbeing and independence, helping to avoid unnecessary admission to hospital.
- Deliver exceptional end of life care including symptom management and help in identifying and achieving people’s preferences.

Requirements
- Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of.
- We welcome applications from newly qualified nurses/occupational therapists/physiotherapists.
- A comprehensive and fully supported Preceptorship Programme for newly qualified healthcare professionals is provided.
