Role Overview
This role is to cover maternity leave until 30/08/2027. Working 22.5 hours per week, the shift pattern is over 7 days a week and will include regular evening shifts and may be required to cover night shifts to cover sickness and annual leave as the need arises.
Key Responsibilities
- Work as a registered Nurse within Community Services across the Dalton and Ulverston District nursing footprint.
- Provide evidence-based, holistic nursing care of patients.
- Collaborate with the integrated team across community settings such as patients’ homes, residential and nursing homes.
- Support the Sister/Team Leader in leading the team, supervising, directing staff, and delegating workload according to the skills of the staff within the team.
- Develop the ethos of integrated care and seamless services ensuring that patient care is delivered to the highest possible standard.
Job Duties
- Deliver a wide range of highly skilled interventions in the patients' own home.
- Manage long-term conditions including diabetes care, end of life care, catheter care, and symptom control and support.
- Support patients to remain in their own homes for as long as possible, preventing hospital admission and supporting patients to lead a long and happy life.

Working Environment
- Operate from three main hospitals: Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal.
- Work from a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.
Hospital Services
- FGH and the RLI provide General Hospital services, with full Emergency Departments, Critical/Coronary Care units, and various Consultant-led services.
- WGH provides General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life-threatening conditions such as broken bones and minor illnesses.

Planned Care
- All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case, and inpatient surgery.
- A range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
Community Services
- Provide Community Service for adults in people's homes, community centres, clinics, GP Practices, and community hospitals.
- Work with people to help them remain independent, improve their health, and manage their conditions through high-quality care, advice, and support.
